Playcious Waiver

PLEASE READ CAREFULLY

I acknowledge that participating in Playcious programs, parties and/or use of the play area and trampoline equipment, creates a risk of injury, property damage, and death. To help ensure safety and prevent injuries, Playcious is a parent-supervised facility.Parents and guardians must closely supervise their children at all times. It is mandatorythat all children older than 1 year, wear Grip Socks.


RELEASE OF LIABILITY, WAIVER OF CLAIMS AND IDENTITY AGREEMENT

In consideration of Playcious accepting my application to participate in its programs, I agree: To waive any and all claims that I and/or my next of kin, heirs, executors, administrators and assigns may have in the future against Playcious and others. To release Playcious AND OTHERS from any and all liability for any of the risks listed. Playcious is not responsible for any injuries or lost and/or stolen items. I give Playcious permission to use any pictures taken of my child and/or myself for promotional use. Playcious has the right to refuse admission or ask you and your child(ren) to leave if equipment is being used improperly or in an unsafe manner.

  • We are a nut/peanut aware facility! We try our best to keep Playcious nut free but we cannot make any guarantees.
  • No outside food and drinks allowed.
  • Adults are REQUIRED to wear socks (slippers, indoor shoes are PROHIBITED).
  • Children are REQUIRED to wear grip socks.
  • Strollers are prohibited from being taken into the play area.
  • For those wearing dresses/skirts, Playcious recommends you wear tights/leggings to help prevent injuries.
  • Please remember that children/students under 18 must be accompanied by a person18 years or older.
  • Playcious reserve the right to refuse or cancel admission anytime if it is not compliance to the Playcious Policy, that can be obtained from Playcious.com
  • You may receive specials and coupons from Playcious as a prefered customers.



By signing this document you waive certain legal rights, including the right to sue.
